With the help of an in-built intellectual analysis, doctors can see in real-time whether their patients are stable or at high risk. Depending on the results, the list of patients is sorted from highest to lowest risk of health deterioration.

Patient interface:

Caregiver information - the patient can find information about their caregiver and their institution so they could research them if needed.

Goals - On the main screen patients can see their goals (activity, medication, diet, vitals) so that they can plan and ensure the goals are met after the day ends. They can leave comments and add photos to each goal they are achieving to let their caregiver know. Caregivers leave clear commentary for each goal so that the patient could study them and apply them to their routine.

Profile - each patient can fill in their profile page, their personal and contact details, and select a photo.

Caregiver interface:

Patient list - overview of the most important details of each patient together with risk rate and vitals. A caregiver can click through on any of the patient summaries to view their full profile where they can also find the patient’s risk score, and medication reports, distinguish between not taken, partially taken, and fully taken medication, and see the patient’s food intake.
They can compare recent data with any historic records. If they have PDFs with any client info, they can upload them into the system for future use.
Patient management - caregivers can create a new patient in the system. They enter any unique login (patient ID or email for example) and create a temporary password for the patient so that they can easily set up a new account.
There they can also create appointments for the patient so that they could schedule the next meeting.

Profile - each caregiver can fill in their profile page, their personal and contact details, select a photo, and add information about their degree, educational and working experience.
